Please let us work with you in this manner by reading and filling out a New Patient Form beforehand. The form is fillable, and you can type data into it. Once you have read and completed entering as much information as possible, please print, sign, scan, and e-mail as an attachment, or simply bring the form with you. This simple process will minimize time wasted on filling out information while in the office, and it also allows us to assess insurance benefits and limits so that we may better assist you in coordinating your treatments.
